Best Natural Sweeteners for Boosting Immune Health

The best natural sweeteners are the ones that help you reduce refined and highly processed sugar without simply trading it for another heavily processed product. Our favorites include stevia, raw honey, pure maple syrup, monk fruit, maple sugar, coconut sugar, and date sugar.

Stevia and monk fruit are intensely sweet with little to no glycemic impact when used in pure forms. Raw honey, maple syrup, maple sugar, coconut sugar, and date sugar still contain real carbohydrates and can raise blood glucose, so we use them intentionally. What makes them useful is that they come from recognizable food sources and, depending on the sweetener, can retain naturally occurring minerals, fiber, or plant compounds that refined white sugar does not.

High added-sugar intake is associated with weight gain, dental disease, type 2 diabetes, cardiovascular disease, and other chronic-health concerns. Cancer cells do use glucose, but eating sugar does not selectively “feed” only cancer cells. The more important concern is the metabolic environment that excessive added sugar can help create, including obesity, insulin resistance, hyperglycemia, and chronic inflammation.

We still recommend getting your palate off the constant need for sweetness. Natural sweeteners are tools, not permission to turn every meal into dessert.

By now, everyone should know that too much processed sugar is bad news.

The problem is that many people hear “cut sugar” and immediately replace it with another ultra-processed sweetener, a diet product, or something marketed as healthy simply because it has a low glycemic index.

That is not what we mean.

When we talk about the best natural sweeteners, we mean simple options that can help you move away from refined sugar while still making real food taste delicious.

Our favorites are:

  • Stevia
  • Raw honey
  • Pure maple syrup
  • Monk fruit
  • Maple sugar
  • Coconut sugar
  • Date sugar

Each works differently.

Stevia and monk fruit are high-intensity sweeteners, so you use very little. Honey, maple syrup, maple sugar, coconut sugar, and date sugar are still concentrated sources of carbohydrate, so they need to be used wisely. Date sugar is a little different because a true 100% date sugar is made from ground dried dates and retains whole-fruit solids.

None of these should become an excuse to keep your taste buds trained to expect everything to be sweet.

The real win is learning to enjoy whole foods again.

Current Health Care Crisis

Americans spend an extraordinary amount of money on health care, yet spending more has not automatically translated into better health.

According to the Centers for Medicare & Medicaid Services, U.S. health care spending reached $5.3 trillion in 2024, or $15,474 per person, accounting for 18% of the nation’s gross domestic product. (1)

That should make us think.

We can take every supplement under the sun and bathe ourselves in coconut oil and fish oils, but if the foundation of our diet is highly processed food, sugary drinks, desserts, refined flour, and constant snacking, we are working against ourselves.

Food choices matter.

That is especially true with processed sugars and highly refined carbohydrates because they are easy to overconsume, absorbed quickly, and found in foods that are often low in fiber, protein, and micronutrients.

As a result, a growing number of people are looking for healthy natural sweeteners that taste great and satisfy sugar cravings.

That is a good place to start.

But replacing white sugar with maple syrup does not automatically make a dessert a health food.

The larger goal is to reduce the overall sweetness of the diet, build meals around whole foods, and use natural sweeteners strategically.

Why You MUST Kick the Sugar Habit

Experts also say that “sugar feeds cancer,” but that's not entirely true.

That phrase comes from something very real: cancer cells often consume more glucose than normal cells. Many tumors reprogram their metabolism to take up and use large amounts of glucose to support rapid growth.

But sugar does not selectively feed only cancer cells.

Every cell in your body can use glucose, and your body works hard to keep glucose available in the bloodstream. Even if you stopped eating table sugar completely, your liver could still make glucose from other nutrients because healthy tissues need it too.

The National Cancer Institute puts an important boundary around the claim: cancer cells do consume more glucose than normal cells, but studies have not shown that eating sugar makes a person's cancer worse or that eliminating sugar causes cancer to shrink or disappear. (18)

So why do we still take the phrase “sugar feeds cancer” seriously?

Because the metabolic environment matters.

A diet overloaded with added sugars can contribute to excess body fat, insulin resistance, hyperglycemia, and chronically elevated insulin. NCI notes that obesity is associated with increased risk for multiple cancers and that people with obesity often have higher insulin and insulin-like growth factor-1 levels, signaling pathways that can influence cell growth. (19)

There is also fascinating mechanistic research showing that specific sugars may influence tumor growth through pathways more complicated than simply giving cancer cells glucose. In 2025, NCI highlighted laboratory research showing that excess fructose could promote tumor growth indirectly after the liver converted fructose into lipids that cancer cells could use. (20)

That is why I would not throw the phrase away.

“Sugar feeds cancer” is a useful warning when we understand what it means. Cancer is metabolically hungry, abnormal glucose metabolism is a hallmark of many tumors, and a chronically high-sugar diet can help create a metabolic environment we do not want.

But it is not as simple as saying that one spoonful of sugar travels straight to a tumor while healthy cells ignore it.

The bigger concern is what a high-added-sugar dietary pattern can do over time.

Excessive sugar intake can contribute to:

  • Weight gain and obesity
  • Insulin resistance
  • Chronically elevated glucose and insulin
  • Metabolic dysfunction
  • Inflammatory signaling
  • Greater intake of ultra-processed foods

Those metabolic changes matter because obesity, insulin resistance, and metabolic disease are associated with increased risk for several cancers.

A 2025 comprehensive review of dietary sugars and cancer concluded that high sugar intake may contribute to cancer risk through pathways involving insulin resistance, chronic inflammation, oxidative stress, and altered cellular metabolism. (2)

Studies of sugar-sweetened beverages also continue to find associations with several adverse health outcomes. A 2024 umbrella review found that most evaluated analyses linked greater sugar-sweetened beverage consumption with poorer health outcomes. (3)

So no, we do not need to pretend that one cookie directly pours fuel into a tumor.

But we also do not need to pretend that a diet dominated by processed sugar is harmless.

We recommend getting it out of the center of your diet.

Is Sugar Really Addictive?

Consuming processed sugar and refined carbohydrates can create a powerful reward cycle.

It's been said that “sugar is more addictive than cocaine.”

That sounds sensational, but the comparison did not come out of nowhere.

In animal choice experiments, researchers found that intense sweetness could be more rewarding than cocaine. In one widely cited rat study, most animals preferred sweetened water over intravenous cocaine, even after drug sensitization. (21)

A later review by addiction researcher Serge Ahmed concluded that sugar and intense sweetness can substitute for addictive drugs in animal models and, under certain experimental conditions, can be more rewarding and attractive than cocaine. (22)

Other researchers have documented addiction-like patterns with intermittent sugar exposure, including bingeing, withdrawal-like behavior, craving, cross-sensitization, and repeated dopamine and opioid signaling in reward pathways. (23)

So there is legitimate science behind the comparison.

At the same time, we should not pretend that a rat choosing saccharin or sucrose over cocaine proves that ordinary sugar consumption produces the same clinical addiction as cocaine in humans.

Human evidence is more complicated.

A scientific review specifically examining “sugar addiction” found little evidence that sugar by itself causes a substance-use disorder in humans independent of highly palatable foods and eating patterns. (4) Other researchers argue that there are substantial behavioral and neurochemical parallels between sugar and drugs of abuse. (24)

That debate does not make sugar cravings imaginary.

Anyone who has tried to stop eating cookies, sweetened coffee, soda, or dessert every night knows how powerful the pull can feel.

So I am comfortable saying, “It's been said that sugar is more addictive than cocaine.” The science gives us a good reason to take the comparison seriously, as long as we explain that the strongest head-to-head evidence comes from animal models and that human addiction science is still debated.

The practical solution remains the same: reduce exposure, build meals around protein, healthy fats, vegetables, fiber-rich foods, and whole-food carbohydrates, and give your taste buds time to reset.

How Sugar Affects Immunity

The original article said that 100 grams of sugar can “shut down” the immune system by 50% to 90% for several hours.

That wording overstates what the underlying research demonstrated.

The foundational 1973 study did find that consuming 100 grams of glucose, fructose, sucrose, or honey temporarily reduced a measure of neutrophil phagocytic activity. The study did not show that the entire immune system shut down. (5)

That distinction matters.

Your immune system is a vast network of white blood cells, antibodies, signaling molecules, tissues, barriers, and organs. One laboratory measure of neutrophil activity is not the same as measuring the whole immune system.

At the same time, there is solid evidence that hyperglycemia can interfere with innate immune function.

Reviews of short-term high blood glucose show changes in neutrophil migration, phagocytosis, oxidative burst, microbial killing, and other aspects of host defense. (6)

So our conclusion has not flipped.

A diet that repeatedly drives poor glucose control is not something we recommend for immune health.

We simply describe the mechanism more accurately now.

Instead of saying one soda “turns off half your immune system,” think of it this way: repeated high-glucose exposure and metabolic dysfunction can impair several immune functions, and that is one more reason to move away from a high-sugar diet.

Do Natural Sweeteners “Boost” Immunity?

Some natural sweeteners contain interesting plant compounds.

Raw honey contains phenolic compounds and has antimicrobial properties. Maple syrup contains polyphenols. Stevia and monk fruit contain unique glycosides that have been investigated for antioxidant, metabolic, and anti-inflammatory effects.

That does not mean a spoonful of any sweetener acts like an immune treatment.

The biggest immune benefit may be what happens when these options help you eliminate a much larger amount of refined sugar and ultra-processed food.

That is the context that matters.

More Risks With Sugar Than Expected

Sugar is everywhere.

That is the real problem.

Added sugar can hide in:

  • Sauces
  • Salad dressings
  • Breads
  • Breakfast cereals
  • Flavored yogurt
  • Coffee drinks
  • Protein bars
  • “Healthy” granola
  • Sports drinks
  • Condiments

The strongest evidence does not support a list of 141 separate diseases supposedly caused by sugar.

It does support taking high intake seriously.

A 2024 review summarizing systematic-review evidence connected high sugar intake with dental caries, unhealthy weight gain, type 2 diabetes, cardiovascular disease, and other metabolic concerns. (7)

The point is not to blame every symptom on sugar.

The point is to recognize that a high-added-sugar diet can sit underneath several metabolic problems at once.

How Much Added Sugar Is Too Much?

The World Health Organization recommends reducing free sugars to less than 10% of total energy intake and suggests that reducing them below 5% may provide additional health benefits. (8)

We use that as context, not as a goal to see how much sugar we can “get away with.”

If you can train your family to enjoy foods that are less sweet, do it.

Fruit should taste sweet.

Carrots should taste sweet.

A cup of tea does not need to taste like dessert.

That palate reset is one of the most valuable things you can do.

Glycemic Index and Why It Matters

A lot of so-called “best natural sweeteners” have gained fanfare because they are low on the glycemic index.

The glycemic index, or GI, ranks carbohydrate-containing foods according to how much they raise blood glucose after eating compared with a reference food.

The 2021 international GI tables remain one of the most comprehensive collections of standardized glycemic-index testing. (9)

  • Low GI: 55 or less
  • Moderate GI: 56 to 69
  • High GI: 70 or above

But GI is not the whole story.

A food can have a low GI because it contains a lot of fructose, which does not raise blood glucose as rapidly as glucose. That does not automatically make it a better food.

GI also does not tell you:

  • How processed the product is
  • How much you will eat
  • How much fructose it contains
  • Whether it contains beneficial nutrients
  • Whether it causes digestive symptoms
  • Whether the product is mixed with other sweeteners

Glycemic load matters too because it considers both the GI and the amount of carbohydrate consumed.

That is why we do not choose a sweetener from one number alone.

Stevia and monk fruit work well because pure extracts contribute little usable carbohydrate in the amounts needed for sweetness.

Honey and maple syrup contain real sugar, so use them as real sugar, not as unlimited “health food.”

Best Natural Sweeteners and Healthy Sugar Alternatives

When it comes to eating the right foods, what you use matters just as much as what you remove.

Thankfully, the best natural sweeteners and healthy sugar alternatives are easy to find.

These are the options we reach for most. Stevia and monk fruit are useful when we want sweetness without a meaningful sugar load. Honey, maple syrup, maple sugar, coconut sugar, and date sugar are better suited to recipes that need the flavor, bulk, browning, or texture of a real caloric sweetener.

1. Stevia

By far one of our favorite natural sweeteners, stevia comes from Stevia rebaudiana, a South American plant whose steviol glycosides are intensely sweet.

Pure high-purity steviol glycosides contain essentially no usable calories in ordinary sweetening amounts and have little direct impact on blood glucose.

That makes stevia especially useful when you are trying to reduce sugar without giving up sweetness altogether.

What Does the Human Research Show?

The original article said stevia had been shown to reverse diabetes and obesity.

That claim is stronger than the clinical evidence.

A 2024 meta-analysis found that stevia consumption was associated with lower fasting blood glucose in some analyses, particularly over shorter study durations, but did not significantly improve HbA1c or insulin overall. The certainty of evidence was low to very low. (10)

That is still encouraging.

Stevia is valuable first because it can replace a meaningful amount of added sugar without adding another meaningful sugar load.

There is also interesting preclinical cancer research.

The breast-cancer study cited in the original article found that stevioside affected breast cancer cells in laboratory conditions. That is a cell-culture finding, not evidence that stevia prevents or treats breast cancer in people. (11)

Time will tell how much therapeutic potential compounds from this plant have.

For now, we use stevia because it is practical, potent, plant-derived, and extremely useful for reducing refined sugar.

Read the Stevia Label

One of the biggest mistakes people make is assuming that every product with the word “stevia” on the front is pure stevia.

Some powdered products are mostly erythritol, dextrose, maltodextrin, or another bulking ingredient with a small amount of stevia added.

Read the ingredient list.

If you are choosing stevia because you want stevia, buy stevia.

2. Raw Honey

Raw, unfiltered honey has something table sugar does not: a complex mixture of naturally occurring plant compounds, organic acids, enzymes, and small amounts of minerals.

Honey can contain amino acids and micronutrients such as potassium, calcium, magnesium, iron, and zinc, although the amounts vary and are generally modest at normal serving sizes.

If you are specifically working on nutrients like magnesium or zinc, whole foods rich in those minerals will contribute much more than a teaspoon of honey.

Honey’s strongest clinical claim is not that it is a vitamin pill.

It is wound care.

A 2024 systematic review and meta-analysis of chronic wounds found that honey dressings improved several wound-healing outcomes, although the authors rated the overall evidence quality very low because of bias, inconsistency, and study limitations. (12)

That is why medical-grade honey has a legitimate place in wound-care research.

What About Local Honey for Seasonal Allergies?

You have probably heard the idea that taking a teaspoon of local raw honey helps your body build tolerance to local pollen.

The research is not strong enough to promise that.

The birch-pollen study cited in the original article used birch pollen honey, meaning honey that had birch pollen deliberately added to it. That is different from simply buying any jar of local honey. Participants using the pollen-enriched honey reported fewer symptoms and medication days than controls, but it was a small pilot study. (13)

So enjoy local raw honey.

Buy from beekeepers you trust.

Just do not tell someone with serious pollen allergies that ordinary local honey is proven immunotherapy.

What About Manuka Honey?

If you can afford it, get some Manuka honey.

It is more expensive, but its unique antimicrobial chemistry has made it one of the most researched honeys for topical and medical applications.

For everyday sweetening, a quality raw local honey is perfectly reasonable.

And remember: honey is still sugar.

Use it because you prefer the whole-food profile and flavor, not because the word “raw” makes unlimited spoonfuls disappear metabolically.

3. Maple Syrup

Pure maple syrup is a delightful replacement in baked goods, sauces, dressings, and other recipes that need a liquid sweetener.

Maple syrup contains minerals, especially manganese, along with a variety of phenolic compounds.

For years, most of the exciting maple research was laboratory or animal work.

Now we have a human trial.

A 2024 randomized, double-blind, controlled crossover study in adults with mild metabolic alterations compared replacing 5% of daily calories from refined sugar with an equivalent amount of maple syrup for eight weeks. Maple syrup modestly improved several cardiometabolic measures, including abdominal fat and glucose-related outcomes, compared with refined sugar. (14)

That is exactly the kind of comparison we care about.

The study did not show that maple syrup is sugar-free or that you can consume unlimited amounts.

It showed that when people replaced refined sugar with an equivalent amount of maple syrup, some metabolic markers moved in a favorable direction.

That supports what we have taught for years: if a recipe needs a caloric sweetener, real maple syrup is one of the better choices.

Buy the Real Thing

There is a huge difference between pure maple syrup and “pancake syrup.”

Read the label.

You want maple syrup, not corn syrup, caramel color, artificial flavor, and a picture of a maple leaf on the bottle.

We buy our maple syrup a few gallons at a time in Michigan at a Farmer's Market near my wife's old hometown when we visit family. Try to get yours local and organic if possible.

4. Monk Fruit (Luo Han Guo)

If you’re looking for a natural sweetener that is as close to guilt-free as it gets, monk fruit may become one of your favorites.

Native to southern China, monk fruit, also known as Luo Han Guo or Siraitia grosvenorii, gets its intense sweetness from compounds called mogrosides.

Pure monk fruit extract contributes very little sugar at the tiny amounts used for sweetening, so it has minimal glycemic impact.

What Does Human Research Show?

In a randomized crossover study comparing beverages sweetened with sucrose, aspartame, stevia, and monk fruit, the nonnutritive sweeteners produced much lower immediate glucose and insulin exposure than the sucrose drink. Differences in total daily energy intake were smaller because participants compensated later in the day. (15)

That is useful context.

Monk fruit can help remove a large sugar dose from a recipe or beverage, but it does not automatically control everything you eat afterward.

The original article described monk fruit as an immune-boosting, antibacterial sweetener.

Mogrosides do have interesting antioxidant, anti-inflammatory, and antimicrobial findings in preclinical research, but robust human immune studies are still limited.

So we recommend monk fruit primarily because it is a powerful plant-derived sweetener that can replace sugar with almost no glycemic load.

Watch the Blends

Just like stevia, commercial monk fruit products are often blended with erythritol or other bulking sweeteners.

If you want pure monk fruit, read the label.

FDA has reviewed multiple GRAS notices for monk fruit extracts used as sweeteners and has issued “no questions” responses to the safety conclusions under the specified conditions of use. (16)

Honorable Mention: Blackstrap Molasses

Blackstrap molasses deserves an honorable mention because it retains more minerals than ordinary refined sugar.

It can contribute iron, potassium, calcium, magnesium, and other minerals, depending on the product.

The older version of this article described molasses as having established “infection-fighting” and anti-tumor effects.

Laboratory research on compounds found in molasses is interesting, but that wording goes beyond human evidence.

Use molasses because it is intensely flavorful, mineral-containing, and useful in specific recipes.

Make sure your molasses is organic and unsulphured.

5. Maple Sugar

Maple sugar is one of my favorite ways to get the flavor of maple syrup when a recipe needs a dry, granulated sweetener instead of extra liquid.

Traditional granulated maple sugar is made by taking pure maple syrup and continuing to cook and work it until the concentrated syrup crystallizes into granules. Cornell’s Maple Program teaches granulated maple sugar as one of the classic confections made directly from maple syrup. (25)

That means maple sugar comes from the same maple sap as pure maple syrup, just in a different form.

I like it because:

  • It has a rich maple flavor.
  • It works well in cookies and other baked goods that need a dry sweetener.
  • It comes from a single recognizable ingredient when you buy 100% pure maple sugar.
  • It is easy to blend with other whole-food sweeteners.

Maple sugar is still concentrated sugar, primarily sucrose, so we do not treat it as a free food.

The encouraging human research discussed above was done with maple syrup, not granulated maple sugar. In that 2024 controlled trial, replacing refined sugar with an equivalent amount of maple syrup produced favorable changes in several cardiometabolic measures. (14)

Because maple sugar comes from concentrated maple syrup, it shares the same botanical source, but we should not automatically claim that every clinical result from maple syrup has been proven for maple sugar.

For us, the reason to choose maple sugar is simpler: it is a minimally complicated, flavorful alternative to refined white sugar when a dry sweetener works best.

6. Coconut Sugar

Coconut sugar, sometimes called coconut palm sugar, is made from the sap of coconut palm blossoms rather than from the coconut meat itself.

The sap is collected and heated until much of the water evaporates, leaving a brown granulated sweetener with a deep caramel flavor.

A 2023 review of coconut sugar found that it is composed largely of sucrose, with smaller amounts of glucose and fructose, and also contains minor amounts of minerals, phenolic compounds, and other plant constituents. The review also reported that published GI values for coconut sugar have generally been lower than those for pure sucrose. (26)

That sounds great, but keep the bigger picture in mind.

Coconut sugar is still sugar.

It is not a zero-glycemic sweetener, and the trace nutrients in a normal serving are not a reason to eat unlimited amounts.

I like coconut sugar because:

  • It is less refined than ordinary white table sugar.
  • It has a rich brown-sugar-like flavor.
  • It works beautifully in cookies, muffins, sauces, and other baked goods.
  • It blends especially well with date sugar and maple sugar.

If you use coconut sugar, look for a product with one ingredient: coconut palm sugar.

Do not let “coconut” on the label convince you it behaves like coconut meat or coconut oil. It is a sweetener, and we use it like one.

7. Date Sugar

Date sugar is one of the most interesting options on this list because true date sugar is not simply purified sugar extracted from a fruit.

It is made by drying dates and grinding the whole fruit into a powder.

That means a 100% date sugar can retain some of the fiber, minerals, and polyphenols naturally found in dates instead of stripping the fruit down to purified sucrose.

Human studies on whole dates show that their glycemic response varies considerably by variety and ripeness. A 2023 review reported GI values ranging from the low 40s into the 70s across different dates and preparations. (27)

That does not mean every date sugar product has the same GI as a whole date.

Grinding, drying, particle size, serving size, and the rest of the meal can all change the glucose response.

Still, date sugar gives us something refined white sugar does not: the solids from the whole fruit.

I especially like it for:

  • Cookies
  • Muffins and quick breads
  • Crumb toppings
  • Oatmeal
  • Homemade granola
  • Blended natural-sugar mixtures

Because date sugar contains fruit solids, it does not dissolve into liquids exactly like white sugar. That is one reason I prefer it in baking or in a blend with other natural sugars.

Look for a product made from 100% dates without added cane sugar, fillers, or syrups.

Bonus: Mama Z's Christmas Cookie Sugar Blend

One of my favorite ways to use maple sugar, coconut sugar, and date sugar is to blend them together.

This gives you the caramel notes of coconut sugar, the rich flavor of maple sugar, and the whole-fruit character of date sugar in one easy baking blend.

I make a big batch and keep it in a glass jar so it is ready when we are baking, especially around Christmas.

Christmas Cookie Sugar Blend

Ingredients

  • 6 ounces organic coconut sugar
  • 11-16 ounces organic date sugar (based on your available container sizes)
  • 6 ounces organic maple sugar

Supplies

  • Gallon-size Ziploc bag
  • Blender
  • Glass jar for storage

Instructions

  1. Place the coconut sugar, date sugar, and maple sugar in a gallon-size Ziploc bag. Shake to mix thoroughly.
  2. Place mixture in a blender and blend until powdery. I prefer milling this recipe in a Vitamix dry goods container. I start blending on one, then slowly increase until the mixture is fully blended. Then, I open the blender, stir, and repeat the process. The final color of your product will differ based on the brand of sweetener you use. Use in place of regular sugar in all your favorite baked goods. Store in a glass jar.

This is a real-sugar blend, so it is not the same thing as using stevia or monk fruit.

The point is not that coconut, date, and maple sugars somehow stop being sugar when we mix them together. The benefit is that when a recipe truly needs granulated sugar for structure, texture, or browning, this gives us a whole-food-based alternative to refined white sugar that tastes incredible.

What About Agave & Sugar Alcohols?

Both agave and sugar alcohols like xylitol have been praised because they tend to produce a smaller immediate glucose rise than table sugar.

That does not make them our preferred choices.

Why We Avoid Agave

Agave became popular because of its low glycemic index.

But much of that low GI comes from its high fructose content.

Fructose does not raise blood glucose as rapidly as glucose, so the GI number can look impressive even when the product is still a concentrated added sugar.

That is exactly why GI cannot be the only test.

We would rather use a little raw honey or maple syrup when a recipe needs real sugar, or stevia and monk fruit when it does not.

What About Xylitol?

We have a full article on xylitol because this sugar alcohol is more complicated than its marketing suggests.

The original version of this article said xylitol cannot be properly metabolized by humans and gets stored in the liver, eventually causing liver damage.

That is not an accurate description of normal human xylitol metabolism.

Humans can metabolize xylitol, and the most common established side effects from larger amounts are gastrointestinal, including gas, bloating, and diarrhea.

However, newer research has created a different concern.

A 2024 study found that higher circulating xylitol levels were associated with a greater three-year risk of major adverse cardiovascular events in high-risk patients. Experimental work in the same paper found that xylitol increased platelet responsiveness and thrombosis potential. (17)

That is concerning, but it does not prove that ordinary dietary xylitol causes heart attacks.

The observational portion cannot establish cause and effect, and researchers are still debating how these findings should affect real-world intake.

For us, it is another reason not to make sugar alcohols the foundation of a “healthy” diet.

And the Other Sugar Alcohols?

Sugar alcohols such as sorbitol, maltitol, erythritol, and xylitol are incompletely absorbed to varying degrees.

That is part of why they often have a lower glycemic impact.

It is also why they can cause digestive distress.

We have discussed sorbitol concerns separately.

Our position is simple: a lower number on a glycemic chart does not automatically make a highly processed sweetener our first choice.

We would rather retrain the palate and use simpler ingredients.

Glycemic Index of Common Sweeteners

The original article included a giant table assigning a single GI value to dozens of sweeteners.

That looks precise, but glycemic index does not work that neatly.

GI varies by product composition, testing method, serving, and formulation. Some high-intensity sweeteners such as stevia and monk fruit contain so little available carbohydrate at ordinary serving sizes that a conventional GI test is not especially meaningful.

So this updated table focuses on the sweeteners people actually ask us about.

The 2021 international GI tables remain a valuable reference, but the best decision is rarely made by GI alone. (9)

  • How processed is it?
  • How much am I actually eating?
  • Does it contain real nutrients or plant compounds?
  • Does it agree with my digestion?
  • Is it helping me reduce my overall need for sweetness?

What are the best natural sweeteners?

Our favorites are stevia, raw honey, pure maple syrup, monk fruit, maple sugar, coconut sugar, and date sugar. Stevia and monk fruit are especially useful when you want sweetness with minimal glycemic impact. Honey and maple syrup work well when a recipe needs a liquid caloric sweetener, while maple sugar, coconut sugar, and date sugar are useful when baking requires a dry sweetener.

What is the healthiest substitute for sugar?

There is no single best option for every recipe. For drinks and recipes where you only need sweetness, stevia or monk fruit can dramatically reduce added sugar. For baking and sauces where sugar contributes bulk, moisture, or browning, raw honey or maple syrup may work better.

What is maple sugar?

Maple sugar is a granulated sweetener made from pure maple syrup that is cooked further until it crystallizes. It gives baked goods a rich maple flavor without adding the extra liquid that maple syrup would.

Is coconut sugar healthier than white sugar?

Coconut sugar is less refined and contains small amounts of minerals and plant compounds that ordinary white sugar does not. Some published tests have also reported a lower GI than sucrose. It is still largely sugar, however, so we use it as a better ingredient choice rather than an unlimited health food.

Is date sugar healthier than white sugar?

A true 100% date sugar is made from ground dried dates, so it retains whole-fruit solids, including some fiber and polyphenols. It is still a concentrated carbohydrate and can raise blood glucose. We especially like it for baking and blending with coconut and maple sugar.

What is Mama Z's Christmas Cookie Sugar Blend?

It is our bonus baking blend made from 6 ounces organic coconut sugar, 11-16 ounces organic date sugar, and 6 ounces organic maple sugar. The mixture is blended until powdery and stored in a glass jar for use in favorite baked goods.

Does honey count as added sugar?

Yes. Once honey is added to a food or beverage as a sweetener, it contributes free or added sugars even though it is natural and contains beneficial plant compounds. We prefer raw honey to refined table sugar in many situations, but we still use it intentionally.

Is maple syrup healthier than white sugar?

Pure maple syrup contains minerals and polyphenols that refined table sugar does not. A 2024 controlled crossover trial also found favorable changes in several cardiometabolic markers when participants replaced an equivalent amount of refined sugar with maple syrup. It is still a concentrated sugar source, so the goal is replacement, not unlimited consumption.

Does stevia raise blood sugar?

Pure steviol glycosides contribute essentially no available carbohydrate in ordinary sweetening amounts, so they do not directly raise glucose the way table sugar does. Some commercial stevia products contain dextrose, maltodextrin, or other ingredients that can change the answer.

Does monk fruit raise insulin?

Pure monk fruit extract has minimal available carbohydrate and generally has little immediate glycemic effect. A small human crossover trial found much lower glucose and insulin exposure after a monk-fruit-sweetened beverage than after a sucrose-sweetened beverage.

Is agave healthier because it has a low GI?

Not in our opinion. Agave often has a low GI because much of its sugar is fructose. A low glucose spike does not tell you everything about a concentrated sweetener’s metabolism.

Are sugar alcohols natural?

Some sugar alcohols occur naturally in small amounts in foods, but the concentrated ingredients used in packaged products are generally manufactured or highly processed for commercial use. That distinction is one reason we prefer simpler sweeteners.

Is xylitol toxic to humans?

Normal dietary xylitol is not accurately described as a poison that humans cannot metabolize. Large amounts commonly cause gastrointestinal symptoms. A 2024 study also raised concern about platelet activation and cardiovascular-event associations, but more research is needed before those findings can be translated into a definitive causal claim.

Does sugar shut down the immune system?

Not literally. A small 1973 study found reduced neutrophil phagocytic activity after large carbohydrate loads, and modern research shows that hyperglycemia can impair several aspects of innate immune function. That is different from saying the entire immune system shuts off for several hours.

Does sugar feed cancer?

Cancer cells use glucose, often at high rates, but so do healthy cells. Eating sugar does not selectively feed only tumors. The more important concern is that excessive added-sugar intake can contribute to obesity, insulin resistance, high insulin, and metabolic dysfunction, all of which may influence cancer risk and progression.

Can natural sweeteners support immune function?

Some contain compounds with antioxidant, antimicrobial, or anti-inflammatory activity, especially honey and plant-derived extracts such as stevia and monk fruit. Human immune evidence varies considerably. The clearest benefit comes when natural sweeteners help you reduce refined sugar and ultra-processed food.

Should I replace all sugar with natural sweeteners?

Replacing refined sugar is helpful, but the larger goal is reducing how sweet your diet needs to be. Let fruit taste sweet. Drink unsweetened water and tea. Build meals around whole foods. Then use natural sweeteners as tools rather than necessities.

Final Thoughts on the Best Natural Sweeteners

Let common sense be your guide when it comes to choosing sweeteners.

You do not need a chemical laboratory in your pantry.

You also do not need to fear every gram of carbohydrate.

The goal is to get processed sugar and ultra-processed sweets out of the center of your family’s diet and replace them with better choices when sweetness is genuinely needed.

For us, that means:

  • Stevia when we want intense sweetness without adding sugar.
  • Raw honey when we want a traditional whole-food sweetener with unique plant compounds.
  • Pure maple syrup when a recipe needs a rich liquid sugar and maple flavor.
  • Monk fruit when we want another plant-derived zero-sugar option.
  • Maple sugar when we need a dry granulated sweetener with maple flavor.
  • Coconut sugar when we want a caramel-like granulated sugar for baking.
  • Date sugar when we want a whole-fruit-based dry sweetener that retains date solids.

And when holiday baking calls for a granulated sugar replacement, we love combining maple sugar, coconut sugar, and date sugar in Mama Z's Christmas Cookie Blend.
We still avoid making agave, artificial sweeteners, or sugar alcohols our everyday solution.

Most importantly, we do not want to swap one sugar dependency for another sweetener dependency.

Retrain your palate.

Cook real food.

Read ingredient labels.

Use sweetness on purpose.

That is how natural sweeteners become part of a healthier life instead of just another way to keep the sugar habit alive.
